2021/2022 Playoff Election Form

Complete our online Playoff Election form below for the 2021/2022 Season Playoffs. All teams must submit this form to determine what playoff division they would prefer to play.

*** IF YOUR TEAM DOES NOT COMPLETE A FORM, YOU WILL NOT BE ENTERED INTO THE PLAYOFFS  ***

Teams entering alternative divisions must meet that event’s player qualifications based on their registered players. Note that Thursday Men’s teams cannot elect to play in the Open playoffs.

Teams that do not qualify will have the opportunity to compete in the “Best of the Rest” playoffs.

Deadline for submissions is Monday, February 28, 2022.
